OUR CENTER

At Kindred Transitional Care and Rehabilitation - San Luis, in Green Bay, Wisconsin, we are able to provide multiple services under one roof. Our short-term rehabilitation unit is on the first floor, with its own separate entrance. There are 15-private suites available in our short-term section, as well as spacious rooms for those in need of more medically complex rehabilitation services.

The second floor of our nursing and rehabilitation center serves as our long-term care area, with three dedicated units. For those who stay with us for a while, we provide we provide safe, compassionate care in a home-like environment that fosters independence and dignity.

We are also home to a Specialized Care Unit that is designed to accommodate 27-individuals with Alzheimer’s or other dementia-related diseases. We are partners with the Alzheimer’s Association, and provide programming and education for both our residents and their caregivers.
