San Luis Medical and Rehabilitation Center accommodates 133 patients and residents. We are located in Green Bay, WI. At San Luis, we strive to meet the needs of our patients and residents and we specialize in dementia care, with our Reflections Unit. Our goal is to provide short-term rehabilitation and sub-acute care in a setting conducive to assisting our patients and residents. We also provide general nursing care for persons with long-term care needs.
Our center includes a separate unit dedicated to the care of those with Alzheimer’s disease and other forms of dementia. Our Alzheimer’s program is designed to celebrate the spirit of these people. We strive to offer a comfortable and compassionate environment that emphasizes positive life experiences and active involvement in a daily routine. The staff who care for those with Alzheimer’s are focused on meeting their special needs. We encourage family to remain integrally involved in the lives of their loved ones and offer ongoing support through individual care plan conferences and support group meetings.
We are conveniently located off Oneida Street, three blocks south of Lambeau Field.
